No it is a pretty lose fit on my daughter.

Q: Would this cap fit a small head size of 21 inches?

I too have a small 21 inch head and it seems to be OK. Most of these types of caps are a stretchy knit so most sized heads can wear them comfortably. I have the black one in the same style as well. I get compliments on them when I wear them. Keep in mind though, these are fairly thin and will not keep your head warm if you need it to in cold weather. I usually have to wear several thin night caps underneath

I have no idea what a kitchen hat is. But Im guessing yes.
